This position involves working as part of the Primary Care Network’s Chronic Disease Management team. The successful applicant will work with family physicians within the Primary Care Network, other health professionals, and local programming to ensure a high quality of care.
Duties and Skills:
- Patient assessments: Assessing the current status of patients with chronic diseases and weight management challenges.
- Providing advanced foot care to patients with chronic disease
- Provide education in a group setting (diabetes, weight management)
- Providing relevant patient education for prevention and management of clinical conditions
- Collaborating and working with a multidisciplinary team including: physicians, nurses, dietitians, exercise specialist, mental health therapist and social worker.
- The ability to undertake disease management, monitor progress, response to treatment and promote disease prevention.
- Any other duties that may be reasonably required for the success of the PCN.
Qualifications:
- Degree/Diploma in Nursing (RN/LPN)
- Experience with chronic disease management would be an asset
- Advanced foot care training would be an asset
- Spirometry training would be an asset
- ABI training would be an asset
- Basic computer skills including word processing
- Ability to use electronic medical records would be an asset
- Strong written and oral communication skills
